To request an extract:
- Download and complete the NR1 form.
- Return the form to us via email or by post. Details on how to do this are included in the form.
You’ll need the following information:
- forename and surname
- date of birth
- date of death
Keep in mind that our research will take into account variations in spellings for surnames.
If the person died outside of Scotland, you must include a copy of their death certificate with your application form.
Prices and payment
The prices per certificate are:
- £15 for the first copy
- £10 for each additional copy
You’ll also need to cover the mailing costs for when we send your certificates to you.
You can pay for your certificates either:
- online, following the instructions in the form
- by cheque or postal order, sent to us by post
Paying online
To pay online:
- Follow the link in the form.
- Select ‘Certificate Extract’ from the list.
If you’re ordering additional copies of your certificate:
- Select the ‘Return to products’ button form your basket
- Select ‘Certificate (Extract) – 2nd+’ from the list, you’ll be able to select the number of additional copies by changing the quantity in your basket.
You can select postage options during checkout.
Once your purchase is complete, you’ll receive an order confirmation number. Make sure to include this number with your completed NR1 form.
Paying by cheque or postal order
Cheques or postal orders must be made out to “Scottish Government”. Send them to us with your completed application form.
The amount included in the check must cover:
- the price of the first certificate
- the price of any additional copies
- postage through Royal Mail
